The D&R application consists of three parts:

  1. Member Statement,
  2. Attending Physician’s Statement, and
  3. Employer Statement.

The initial 119 calendar day period after you become Totally Disabled is known as the Elimination Period. To make a D&R claim, all three parts of the application must be fully completed and submitted to HEB Manitoba within 60 days after the end of the Elimination Period (within 180 days from the Date of Disability).

The sooner your completed claim application is received, the sooner HEB Manitoba can assess your eligibility for D&R Benefits.

Once all claim application forms are completed and the original completed claim application forms have been received by HEB Manitoba your claim will be assessed. If there is additional information that is pertinent to your claim, please provide a copy with your application.

Note:

  1. Under the terms of the Plan, a late application will be denied unless the member provides a written explanation, satisfactory to HEB Manitoba, as to why the application was not received within 60 days after the end of the Elimination Period (within 180 days from the Date of Disability).
  2. Original completed, signed and dated forms must be returned to HEB Manitoba. Photocopies will not be accepted. Exception: HEB Manitoba will accept photocopies of Proof of Age documents.

HEB Manitoba recommends that you complete and submit your application as soon as possible to avoid delays in assessing your claim.

You must re-apply for D&R Benefits (the three parts of the application forms will need to be completed and submitted to HEB Manitoba) for a Recurrent claim. For example, if you received D&R Benefits, returned to work, and then had to stop working again due to the same or a related disability within six months, your claim may be considered a Recurrent claim.
